PCL reconstruction is a specialized knee surgery designed for patients with complete or functionally significant tears of the posterior cruciate ligament, especially when conservative treatment such as physiotherapy, bracing, and activity modification fails to restore stability. The procedure is typically performed using arthroscopic (keyhole) techniques, allowing the surgeon to visualize the joint structures with high precision and minimize tissue disruption. During surgery, tunnels are created in the tibia and femur, and a graft is placed to replicate the natural PCL anatomy and function. The graft gradually integrates with surrounding bone, restoring ligament strength over time. PCL injuries are less common than ACL injuries but can be more complex due to their role in deep knee stability. Patients often present with posterior knee pain, difficulty descending stairs, instability while walking downhill, or a feeling of the knee “giving way.” Postoperative rehabilitation is critical and usually more structured and longer compared to ACL reconstruction, focusing on gradual weight-bearing, controlled motion, and strengthening of surrounding muscles to protect the graft during healing.
